Please enter the required information to set-up your upcoming Pediatric Chemotherapy/Biotherapy Provider Course. Please note that all course set-up forms must be submitted at least three weeks prior to the 1st date of your course. After your course has been approved, registration links and course roster information can be found on the lead instructor’s Instructor Dashboard found in My APHON along with your Provider and APHON Membership Cards.
